Do you need to register, or have Registration questions???
We are currently accepting registrations for the 2020-2021 school year if you live within the Mabry boundary!
What do you need to register?
- Registration Form (SER – Student Emergency Release Form)
- Student’s Birth Certificate OR passport
- FLORIDA immunization record
- FLORIDA physical
- Copy of most recent report card (non-Hillsborough County public schools only)
- Your photo ID (ie: Driver’s License – for identification purposes)
- Student Residency Form with 2 forms of proof
*If you own your home and it is in your name, we can most likely pull the records up online (ie: Homestead Exemption, Warranty Deed, Tax receipt)
*If you have recently purchased a home, and it is not yet reflected online, then you may provide the closing contract for the home, as well as any of the other items mentioned above (Warranty Deed, TECO bill, etc)
If you cannot provide any of these forms, you will need to discuss with the school registrar.
Please fill out the editable Registration Form (SER) & Residency Form; save as a PDF and send via email to the registrar listed. You may send all documents back to the school electronically by: Scan & email – There are MANY apps available that make this easy to do, and right from your phone! (ie: CamScanner, Microsoft Office Lens, Simple Scan, etc). Please save & send as a PDF if possible.
